EvergreenHealth Women’s Urology & Urogynecology Care is committed to helping you navigate today’s ever-changing insurance environment and understand your financial responsibility.
It is important that you understand your policy in advance to be fully aware of the benefits available and any limitations or restrictions that may apply to services you seek. This includes knowing your deductible, copay and co-insurance amounts. We require that all copays and applicable deductibles be paid at the time of your visit. This helps our office to control billing costs and enables us to provide more cost-effective services.
After your initial visit, your physician may make the recommendation that you have further in-office procedures in order to evaluate your condition. Our staff will do its best to provide you with an accurate estimate of anticipated charges and the portion you may be responsible for based on your insurance coverage. We strongly encourage you to also inquire with your insurance carrier prior to undergoing the procedure.
We accept many insurance plans. It is your responsibility to check with your insurance carrier to make sure your plan covers our services and to determine if you need a referral from your primary care physician in order to be covered.
Our staff will provide the information necessary for you to submit charges to your insurance.
Once your insurance company has paid their portion of your claim, you will be responsible for any outstanding balance.
If you are covered by an insurance carrier that we do not accept or are uninsured, payment in full is required at the time of your visit.
